Part Time Outreach Worker

Location: Town of Burlington
The Council On Aging is looking for a part-time Outreach Worker to work with the elder population in Burlington. The position will provide social and emotional services to these elders and their families to promote the well being of seniors. This position will also be working with the Southeast Asian population and unemployed older adults.
 
Duties include, but are not limited to:
  • Conducting in-home visits to assist elders in defining their needs and to facilitate access to services, and follow-up visits to ensure adequacy of assistance.
  • With agreement of elder or family member, makes appropriate referral.
  • Providing advocacy in areas such as legal, social security, food stamps, health, housing, medical and fuel assistance.
  • Assisting with employment searches.
  • Helping to fill out appropriate assistance forms.
  • Seeking/accepting referrals from various sources.
  • Maintaining contact with hospital discharge planners, VNA’s, Minuteman Senior Services and mental health facilities.
  • Keeping accurate and confidential records.

Minimum Qualifications:
Minimum of a high school diploma required. Medical background preferred, along with experience working with the elder population or, alternately, a combination of education and experience deemed equivalent to these. Must have excellent oral and written communication skills, and must also have valid driver's license and access to a vehicle to travel to residences within Town, as well as organizations within and outside of Burlington.
 
Hours are primarily weekdays with some nights and occasional weekend work, approximately 15 to 19 hours per week. Salary is $22.54 per hour. This position does not participate in the Town-benefits program, and is subject to availability of grant funding.

Please note:  MA General Laws mandate that any individual having direct contact at any time with either children or senior citizens be subject to CORI screening by the Criminal History Systems Board.  Any conditional offer of employment is contingent upon satisfactory CORI results.
